Alamo Area Hospice is looking for compassionate and dedicated Licensed Nurses (LVNs/LPNs) to join our team in San Antonio. In this role, you will provide hands-on nursing care and support to patients and their families throughout the hospice journey, helping ensure comfort, dignity, and peace of mind.
Responsibilities:
- Deliver comprehensive nursing care and support throughout the patient’s journey.
- Provide direct patient care, including medication administration and symptom management.
- Collaborate with RNs, the hospice Medical Director, and the interdisciplinary team to carry out individualized care plans.
- Offer emotional support and education to patients and families.
- Maintain accurate documentation in compliance with hospice standards and regulations.
Qualifications:
- Current Texas LVN/LPN license in good standing.
- Minimum of 1 year of nursing experience (hospice or palliative care preferred).
- Compassionate, patient-centered approach to care.
- Strong communication and teamwork skills.
- Reliable transportation for travel within the San Antonio area.
